Below is a list of documents required for registration:
- Pre-Enrollment Card
- Birth Certificate
- Social Security Card
- Immunization Record
- Proof of residence: current electricity bill, rental contract, or contract for new home/deed of trust
- Academic Records (if available)
- Parent/Guardian ID
For Legal Guardianship: Please complete the Official Guardianship Caregiver's Affidavit and include a copy of parent's ID.
For Multi-Family: Notarized affidavit of student residing in a multi-family home with parent/guardian. Please attach current light bill and identification of person listed on bill.
